Did you know that April 16 is National Advance Care Planning Day? This day exists to inspire, educate, and empower the public and providers about the importance of advance care planning.
At this event we ask you to join us to play HELLO, the conversation game that helps you talk about what matters to you, especially when it comes to serious health issues.
